I think all of us were in both sides, so it's easier to feel the insecurities of the other side.
Good callout on the higher-ups - It's definitely much harder. I think as a manager, you should never say it to your people. This can completely break the relationship, you should make them feel confident to make decisions (and mistakes).

The hard part for me was to reconcile how much it annoys me when other people do it, and how often I do it myself... Once I started to notice, I handled both cases much better.

Another one I have seen that gets used - "see you in incident / sev review".
Unfortunately, this behavior is not uncommon. I don't like tolerating it and make it clear to the person to have a discussion rather than say that.

It also creates a bad environment where people are afraid to bring up suggestions and take risks or say an opposing opinion.
